In order to receive a diagnostic mammogram at the Lake Pointe Breast Center, a woman must have an order from her physician. You may bring this in at the time of your visit or your physician can fax it to our center. A physician order is not required for a screening mammogram.
If you do not have insurance and need a referral or resources, please contact our Breast Health Nurse Coordinator at 469-698-7824 to see if you qualify.
Be aware there are many different types of insurance. You are responsible for verifying your breast health coverage and any deductibles or co-pays.
Forms
Please print the forms below and fill them out prior to your appointment, or arrive 20 minutes early to complete the paperwork.
Paperwork is still a normal process in our office. At least 48 hours prior to your first visit, you may pre-register for your appointment on our website – lakepointemedical.com. Or you can print these forms and bring them with you to your first appointment. We will also need you to complete the Patient History Form. If you are an established patient you will need to update the patient history form for each visit.
If you have had breast imaging done previously at another facility, we will need these films for comparison. If you have these, please bring them to your visit. If you need to request these from a facility you may fill out the Request for Imaging Form and we will send in the request.
For patients needing an MRI, the MRI Contrast Safety Screening Form needs to be filled out. If you are a new patient, you will also need to fill out the first visit forms mentioned above.
If you need to have copies of your breast imaging done at the LPBC, please fill out the Release of Medical Information Form form so that we can fulfill your request.

Breast Health for Life
Our goal is early detection of breast cancer through education. We are dedicated to the prevention, screening, diagnosis and treatment of breast cancer. The death rate from breast cancer is going down due to more women receiving mammograms and better treatment of breast cancer. Breast Health for Life is a free educational program presented to community organizations, schools, churches and businesses. The program includes:

Living Well With Cancer
Living Well With Cancer is a monthly program for cancer patients and their families, providing a time to meet and share with others who have cancer, learn how to improve their quality of life, dispel common myths about cancer, and learn more about living with cancer.

Located in the Lake Pointe Imaging Center/Breast Center in Rockwall, the Resource Center is dedicated to helping patients before, during, and after their cancer experience.
Hours are 8:00AM to 5:00PM Monday through Friday.
Breast Health Nurse Coordinator
What is a Breast Health Nurse Coordinator? A compassionate and knowledgeable individual to help guide you and answer your questions related to your breast health and/or breast cancer diagnosis.
“My role as Breast Health Nurse Coordinator is to calmly support the patient and ensure they have all the information they need," explains Terry Pothier, RN. "By providing a personal approach to each patient’s care, I do my best to make a difference in the lives of my patients.”
