Hospitalization creates a wide range of patient issues. It empowers patients to examine and explore emotional, social and financial needs in addition to medical problems. One's ability to function in these areas is important for recovery.

The Social Services Department at Lake Pointe Medical Center is staffed by licensed social workers whose primary focus is to evaluate your potential needs for follow-up care and to facilitate the transition from hospital to home. The department helps you and your family with discharge planning such as home health services, access to an extended-care facility and arranging for medical equipment. They can also arrangeĀ  local transportation and make referrals to appropriate community resources.
