The Social Services Department at Lake Pointe Medical Center is staffed by licensed social workers whose primary focus is to evaluate your potential needs for follow-up care and to facilitate the transition from hospital to home. The department helps you and your family with discharge planning such as home health services, access to an extended-care facility and arranging for medical equipment. They can also arrange local transportation and make referrals to appropriate community resources.
