If you decide to pursue the possibility of Silastic Ringed Vertical Gastroplasty, as a courtesy to you, your insurance company will be contacted to obtain written pre- authorization for your procedure. However, written approval from your insurance company is not a guarantee of payment. The Weight Loss Surgery Program is willing to work with you and your insurance company if you provide the medical/clinical information required by your insurance company. It is important that you review your policy prior to your consultation and consider policy limitations which may include pre-existing conditions or exclusions.  It is your responsibility as the patient to discuss your benefit plan with each doctor that may participate in your care. If an error is made by your insurance company in quoting your benefits, the Lake Pointe Weight Loss Surgery Program will not be held responsible.

 

In the event that your insurance company does not pay, even after written approval, you will be responsible for all charges incurred. Of course, as with any medical services you are responsible for your deductible and your co-insurance amounts.

 

The services of certain program professionals may be necessary before or during your surgery. Please be aware that some insurance policies provide different benefits to hospitals than to physicians. You or your insurance company may be billed separately by any or all of the following:

  • Surgeon                                               
  • Assistant Surgeon                                
  • Anesthesiologist                                   
  • Cardiologist
  • Pulmonologist                                               
  • Internist
  • Radiologist
  • Pathologist
  • Psychologist
  • Laboratory
